Please check your email for confirmation details and complete necessary online forms before your appointment.  We are located on the Corner of Milner Avenue and Central Boulevard (5/2 Central Boulevard, Silverdale) Parking is available in the Rebel/Briscoes Complex.

If you need to change or cancel your appointment, please ensure you give us 24 hours notice or you will incur a $60 cancelation fee.
